• Bank Statement- last 6 month
• Tin Certificate
• Income Tex Certificate
• NID card
• All Bangladeshi treatment documents report
• Documents required for companion: 
• Passport Copy
• Recent Photo.
• Proof of Relationship (Marriage certificate, family certificate etc.)

NB: If you already have an appointment and only need an e-visa, please provide the following documents for the e-visa:  

MHTC Letter. Appointment Letter Passport Copy. Recent Photo. Bank Statement-last 6 month. Tin Certificate. . Income Tax Certificate. Proof of Employment. (NOC/TREAD LICENCE)
